Under 12 Sunday Schedule Changes

Coaches,
After looking at the schedule, the Tournament Committee felt it was to much to ask a team to wait until 2:00, 3:30, or 5:00 on Sunday for a consolation game. We've changed the schedule for Sunday's games to be more accommodating for the consolation games. Due to the fact that we changed the schedule, any semi-final that is tied at the end of regulation will immediately go to penalty kicks. We're sorry for any inconvenience this may cause you or your team. The schedules have been updated on the website to reflect the changes. |

- State Association Certified Team Roster – 2 copies
- Guest Player’s Forms – 2 copies
- Tournament Roster and Players’ Release Form
- Permission or Notification of Travel for out-of-state teams only
- 2008-2009 State Association issued Player Passes with photo, signed, stamped, and laminated
- Individual Medical Release Form
- Visitor Questionnaire Form
